Which examinations in the Prevention 40 PLUS programme
Are you over 40? Take advantage of the package of preventive examinations as part of the Profilaktyka 40 PLUS programme.

The programme is addressed to Poles who:
- are over 40
- have not benefited from the Prevention 40 PLUS programme before
- have completed a questionnaire on the Internet Patient's Account (IKP) or via the infoline 22 735 39 53 or at a participating primary care centre.

How to log in to the Internet Patient Account?

How to use the package
1) Fill in the questionnaire on the Internet Patient's Account (IKP).
2.Wait 2 working days for the e-referral to be issued.
3.Select the facility where you wish to have the examination, contact them and make an appointment for the examination.
4.Bring your identity card to the screening.
5.Pick up the results of the survey at the facility where you performed the service or via IKP.

You can also complete the survey via the helpline: 22 735 39 53 or at one of the participating primary care clinics. The e-referral is valid until the end of 2023.
